Psychogeriatric Resource Consultant (PRC)
The Psychogeriatric Resource Consultant Program (PRC) was developed through the Strategy for Alzheimer Disease and Related Dementia initiative and is sponsored throughout the Kenora/Rainy River Districts by the Canadian Mental Health Association, Fort Frances Branch and Pinecrest Home for the Aged in Kenora.
The PRC provides consultative support to local Long Term Care Facilities and Community Support Agencies who serve people with dementia, mental illness or other complex needs. The PRC has an extensive knowledge base regarding management of mental illness and dementias. This enables them to assist and advise staff in the development and evaluation of care plans for persons who may exhibit behavioural issues.
Additionally, the PRC’s develop and deliver ongoing education and training in the management of individuals with complex, cognitive and mental health needs. This education and training is either care based or general in nature, dependent on the needs of the facility or the agency staff.
The PRC is a part of a variety of geriatric and mental health community committees. This community involvement enables the PRC to operate in the continuum of care and to help in the identification of gaps in service delivery within the Kenora/Rainy River Districts.
The Psychogeriatic Resource Consultant’s role is to:
Provide consultative support to local LTC facilities and community support agencies that serve people who exhibit challenging behaviours
- Inform the LTC/Community Support Services about existing linkages and support
- Provide education and training that enhance local knowledge and skills on dementia
- Assist and advise staff in the development of care plans and interventions for persons with behavioural issues including the use of assessment tools
- Develop an ongoing relationship with LTC and community systems
- Help to identify gaps and integrate services to meet client/staff needs in crisis situations
Services Offered:
- Supports LTC facilities and community support agencies within a designated geographic area on a priority basis, based on risk, urgency and effective use of time
- Acts as an advisor, educator, facilitator and network builder
- Helps to identify gaps and integrate services to meet client/staff needs in crisis situations
- Operates in a continuum of care model and serves as a link between inpatient, Long Term Care Facilities, specialty services and community resources
